14. August 2005: Eine Boeing 737 startet in Zypern nach Athen steigt auf 34.000 Fuß, als ein Alarm die Piloten verwirrt. Sie wussten nicht, dass die Sauerstoffmasken im Passagierraum ausgelöst hatten. Doch die Maschine steigt weiter und die Technik am Boden hat Probleme, von den Piloten eine klare Beschreibung ihrer Probleme zu bekommen. Der Kontakt reißt ab. Nach zwei Stunden steigen Militärjets auf und begleiten die im Autopilot Warteschleifen fliegende 737. Erinnerungen an 9/11 werden wach. Das Militär schießt die Maschine nicht ab. Drei Stunden nach dem Start stürzt die Boeing mit Treibstoffmangel ab. Alle 121 Passagiere an Bord sterben, darunter 22 Kinder. Vom CVR wurde zunächst nur das aufgeschlagene Gehäuse gefunden. Nach einem Problem mit einer hinteren Tür wurde die Anlage für den Kabinendruck auf Manuell gestellt und vergessen, den Schalter wieder auf Automatik zurück zustellen. Der Kopilot versäumte dies beim Abarbeiten seiner Checkliste vor Start ebenfalls. Ein Ermittler begibt sich in eine Unterdruckkammer, um den Hergang des Unfalls zu verstehen. Er kann nachvollziehen, warum die Piloten unter der einsetzenden Hypoxie keine klare Beschreibung ihrer Probleme geben konnten. Ein Flugbegleiter, der über eine Pilotenausbildung für Kleinflugzeuge verfügte, hatte versucht die Maschine zu landen unter Benutzung tragbarer Notsauerstoffflaschen. Er war trainiert, mit wenig Sauerstoff auszukommen. Er hatte sich zu diesem Flug gemeldet, da seine Verlobte als Flugbegleiterin für diesen Flug eingeteilt worden war. Bevor er in benommenem Zustand die Maschine gelandet bekommt, geht der Treibstoff aus. Alle Insassen waren erst beim Aufprall umgekommen. - bsocially

Captain Denny Fitch lost all flight controls in his DC10 but he achieved the impossible by landing his plane on a wing and a prayer.

On 22 November 2003, shortly after takeoff from Baghdad, Iraq, an Airbus A300B4-200F cargo plane was struck on the left-wing by a surface-to-air missile disabling the hydraulic flight control systems. Indonesia AirAsia Flight 8501, an Airbus A320 was a flight between Surabaya and Singapore on December 28, 2014 with 162 people on board. .
The Crash:
The flight left Surabaya 15 minutes late at 5:35am. After departure, the flight was in contact with Jakarta Centre, which provides ATC for most of Indonesia. At 6:12am, the aircraft was flying at 32,000 feet and began to approach violent storms. The pilots contacted ATC to fly a different route and fly at a higher altitude to avoid the storms. ATC deferred Flight 8501 but said they could climb to 34,000 feet but no response was heard. At 6:17am, the aircraft began to pitch upwards from 32,000 feet to 37,000 feet at a rate of 6,000 feet per minute which twice the maximum rate a commercial aircraft should be climbing. At just over 36,000 feet, the jet then began to descend. The plane descended 8,000 feet in half a minute and began to turn left. The airplane then ditched into the sea killing everyone on board. The crash was caused by a rudder failure. The pilots also misunderstood each other during the emergency, causing them to pitch the aircraft up even further, worsening the stall. - bsocially

But when an improperly installed doubler plate causes rapid decompression, the plane crashes into Mount Takamagahara, killing 520 people and becoming the deadliest single-aircraft accident in aviation history.

Kevin and Susan Campbell’s tireless crusade for answers finally results in the NTSB retrieving the cargo door from the ocean floor. This new evidence, combined with the malfunction of another 747’s electrical door locking, leads to the NTSB issuing a revised report. This one concludes that electrical wiring is at fault. Changes are implemented immediately to prevent another accident like Flight 811 from happening.
