Indonesia AirAsia Flight 8501, an Airbus A320 was a flight between Surabaya and Singapore on December 28, 2014 with 162 people on board. .
The Crash:
The flight left Surabaya 15 minutes late at 5:35am. After departure, the flight was in contact with Jakarta Centre, which provides ATC for most of Indonesia. At 6:12am, the aircraft was flying at 32,000 feet and began to approach violent storms. The pilots contacted ATC to fly a different route and fly at a higher altitude to avoid the storms. ATC deferred Flight 8501 but said they could climb to 34,000 feet but no response was heard. At 6:17am, the aircraft began to pitch upwards from 32,000 feet to 37,000 feet at a rate of 6,000 feet per minute which twice the maximum rate a commercial aircraft should be climbing. At just over 36,000 feet, the jet then began to descend. The plane descended 8,000 feet in half a minute and began to turn left. The airplane then ditched into the sea killing everyone on board. The crash was caused by a rudder failure. The pilots also misunderstood each other during the emergency, causing them to pitch the aircraft up even further, worsening the stall. - bsocially

Flight 1420, an American Airlines MD-82, leaves Dallas-Fort Worth for Little Rock two hours late due to thunderstorms. To keep the airline schedules on track, the American Airlines dispatcher tells the crew to “expedite their arrival” to beat the storms to the Little Rock airfield. It’s a short hop and radar weather data shows a clear corridor, or a “bowling alley,” as the dispatcher calls it, through which Flight 1420 should be able to fly to its destination. Captain Richard Buschmann and co-pilot Michael Origel prepare to beat the storm. One hundred miles from Little Rock, as 1420 begins its descent, severe thunderstorms close in on the airfield, bringing with them dangerously unstable winds.

The ensuing accident is the result of bad judgement, decision-making under stress, and one of the most dangerous conditions in modern aviation – “hurry-up syndrome.” Little Rock is a disaster born of the time pressures inherent in modern commercial aviation. Airlines have schedules to keep, and connections to make. Delays can have a ripple effect throughout the system with massive financial consequences. In this case, the rush to land in dangerous conditions has tragic consequences.

When the pilots switched off all non-essential electrical systems, they thought that they had switched off the in-flight entertainment system. As the cockpit fills with smoke, pilots are forced to leave their seats. Investigators believed that there was no one at the controls of Flight 111 when it crashed. When the passengers and crew saw this, they knew they were in trouble.

The Canadian Coast Guard picks up bodies and wreckage on the surface of the water. The black box and the cockpit voice recorder both stopped at the same time as the last transmission. Experts study the remains of the cockpit the discover what caused the fire and it turns out that the in-flight entertainment system provided the spark and the insulation that covers the inside of the aircraft provided the tinder.

PART 2 | Do you think the crash was due to similar causes? A massive salvage operation and the investigation get underway. Ten months earlier, NTSB investigator Dick Rodriguez had investigated another 757 accident. The problem had been a blockage in the pitot-static ports which feed vital flight altitude and speed information to the computers.

When the wreckage of Flight 603 was retrieved from the ocean, the piece of fuselage with the static ports revealed the terrible truth. The static ports were covered up with duct tape that had been put on to try and prevent water from getting inside during the cleaning and wasn't removed before the flight resulting in the crew receiving false information.

The computer sounds an Overspeed warning followed by a stall warning stating that the plane is travelling too slow. The crew become confident of the tower's altitude readings and ignore the ground proximity warning when it appears again. Fooled by the false confirmation of their altitude and unable to see in the dark, the crew crash the plane into the ocean.
