Haben Sie das? Ein Pfeifen im Ohr, ein Piepsen im Ohr oder gar ein permanentes Rauschen im Ohr...dies ist wahrscheinlich ein Tinnitus. Tinnitus ist relativ häufig in Deutschland. Knapp 25% der Deutschen haben mind. einmal in Ihrem Leben einen Tinnitus. Tinnitus kan ein- oder beidseitig auftreten.
Geht man wegen dem Rauschen oder piepsen im Ohr zum Arzt dann macht dieser verschiedene Ursachen. Tinnitus selbst ist aber weniger eine (eigenständige) Erkrankung sondern eher ein sog. Symptom - wie auch akuter Schmerz ein Symptom ist.
Folgende Ursachen für einen Tinnitus gibt es (Auszug):
1. Mögliche Ursachen für objektive Ohrgeräusche (Arzt kann Tinnitus auch hören)
--- bestimmte Tumore (pulsierendes Rauschen)
--- Arterienstenose (= Verengung) (pulssynchrones Rauschen)
2. Mögliche Ursachen für subjektive Ohrgeräusche (Arzt kann Tinnitus nicht hören)
--- Funktionsstörungen des „Hörsystems“
z.B. Entzündliche Prozesse (z.B. Mittelohrentzündung), Lärm (z.B. akutes oder chronisches Schalltrauma), bestimmte Medikamente, Altersschwerhörigkeit, Weitere Pathologien (z.B. geschädigtes Trommelfell, Hörsturz)
--- Ursachen „im“ Kopf oder Gehirn, z.B. Tumore (z.B. Hirntumore),
Operationen (z.B. nach neurochirurgischen Eingriffen oder Ohroperationen), Traumata (z.B. Schädel-Hirn-Trauma, Felsenbeinfraktur)
--- Psychische Belastungssituationen
--- Systemische organische Erkrankungen, z.B. Neurologische Erkrankungen (z.B. Multiple Sklerose), Kardiovaskuläre Erkrankungen (z.B. Bluthochdruck, zu niedrigen Blutdruck, Herzrhythmusstörungen), Stoffwechselerkrankungen (z.B. Schilddrüsenfunktionsstörungen), (Kiefer‑)Orthopädische Erkrankungen (z.B. HWS-Syndrom, Störungen des Kauapparates)
--- Psychiatrische Erkrankungen (z.B. Schizophrenie)

Der Tinnitus selbst wird eingeteilt nach den folgenden Kriterien:
1. Objektivität: objektiver vs. subjektiver Tinnitus
2. Entstehungsort: äußeres Ohr, Mittelohr, Gehirn etc.
3. Verlauf: akut vs. chronisch
4. Schweregrad: Grad 1 = kein Leidensdruck bis Grad 4 = Berufsunfähigkeit

Die Therapie eines Tinnitus erfolgt zumeist wie folgt:
1. Akuter Tinnitus: gefäßerweiternde Infusionen mit und ohne Kortison
2. Permanenter/ häufiger/ chronischer Tinnitus:
--- Grunderkrankung behandeln
--- Begleitende Therapie, z.B. Tinnitus-Counseling = Umgang mit Tinnitus, Hörtherapeutische Maßnahmen, Therapie von Begleiterkrankungen (z.B. Psycho- und/oder Pharmakotherapie), Verhaltenstherapie, Selbsthilfegruppen, Pflanzliche Produkte
